This is what happens when the guide rod is either in the wrong orientation or off-center (on compact models). To me, something looks wrong with the take down lever. Remove slide and make sure the FCU is seated in the grip module and make sure that take down lever is inside properly and press back all the way.

First time cleaning and reassembling, and I think I did two things wrong. The take down lever was in the 6 o’clock position and the guide rod was in the wrong orientation. I said slide lock, not the release. Sounds like a misalignment. Flip up the slide lock and try to man-handle it off by moving it back and forth until it gives, just be careful not to bend it sideways. UPDATE- I got the slide off of the frame but the FCU and the slide are still together and still in the open position.
